There is probably minimal need just about everywhere especially due to the recent news.

I end up writing it in, in the notes. But few people probably see those. I like the idea of showing E85.

Since it's currently an exact rectangle -- each station is assumed to be able to potentially report prices for each type of fuel -- adding a new fuel type that's available at one station would expose a box that can be filled in for *any* station. I think that the danger associated with that change outweighs the benefit of including those few stations that have 4 grades of gas. (Generally, Sunoco.) Likewise for ethanol variants, in my opinion.
This suggestion would be worth revisiting if the presentation is changed so that the input field for a station only accepts fuel types that are available there, according to the station's MSL entry. (A feature that we already want because of the diesel situation.)

I've also seen a few stations that carry four grades of gasoline (87, 89, 91, and 93 octanes, I think). I might be wrong on the exact octane numbers, but my point is that one of those obviously has to be left out under the current system, but which one? And why not just allow all four if a station has them?

I wondered the same thing. I list E-85 pricing in the comments section though, which at least gets the info listed for the station. However, I'm sure many people who would look for this fuel are likely to miss the info since it doesn't actually appear inline with the other fuel types. That's the *BAD* thing about not being able to list it where it should be.
Hopefully a category will be created for E-85 for those stations who sell it so it can be updated and noticed by those who use it.
